Levers lists a one-month NAD+ injection supply at $197, with 1,200 mg for most states and 1,000 mg for California, South Carolina and Indiana. The product page describes 2,400, 4,800 and 7,200 mg for two-, four- and six-month plans outside those three states, with lower quantities in them. Longer plans have split shipments. The visible two-month price is $164/mo, while a selector says $163/mo; generic monthly-billing copy also sits beside multi-month supply choices. Confirm the total charge and delivery schedule for the option you select.
The usable life needs confirmation before buying a larger supply. The product page says 90 days from compounding and at least 60 days from arrival; the return policy says NAD+ has 45 days from compounding and does not guarantee more than 28 days from arrival. Public state lists also disagree: the product lists many states, the shipping page retains an older shorter list, and the terms still contain Arizona-only wording. Ask for current eligibility and the actual vial dates for your order.
Provider support, supplies and shipping are described as included, with no charge unless treatment is approved. Levers distinguishes one-time purchases from subscriptions; the general subscription terms require a two-month minimum and cancellation at least 48 hours before processing. The return page permits refunds before pharmacy processing, then excludes returns for compounded orders placed with a pharmacy. Confirm which agreement applies before paying. The pages checked describe pharmacy standards but do not identify the facility filling your order.
